Please complete the required information below immediately after your event (required information is marked with *).
After completing and submitting the form, you will receive an invoice via the email for you order.
Immediately after payment is received, we will email the individual congratulations emails to you at the email address you provide below in the field titled "Event Contact Person Email". These emails contain all the information necessary for your winning bidders to register their package.
Upon receiving this information, you will forward the information to the individual winners and retain a copy for future reference.
Please have the following information ready before proceeding with the form below:
- Event Name, Organization Legal Name, Organization Billing Address
- Organization Contact Person Name and Email Address
- Event Date
- All Winner Package Codes
- All Winner Last Names
